Initial process of paperwork for what services are available at the address requested. This must be processed before any fees are paid. There is no charge and no obligation for filling out the request. Please fill out, print, and bring into the TUA office at 901 South Jackson Street, Tullahoma, TN 37388. Application for Utility Service
To start the process of application of service, please fill out the form, print, and bring into the TUA office at 901 South Jackson Street, Tullahoma, TN 37388. For questions, contact customer service at 931-455-4515. You can also schedule an appointment with a Customer Service Representative by emailing customerservice@tullahomautilities.com. Please indicate your preferred date (M-F) and time (7:30-3:30) in your correspondence. (See APPLY FOR SERVICE Tab)
Please note that same day service is guaranteed prior to 2:00 p.m.
Bank Draft Authorization
Please fill out all information on Bank Draft Authorization and return to TUA's office with a voided check or copy of a voided check. Please continue to pay until 'Draft' appears under total amount on your monthly statement.
